I would check you don't have a corrupt image or incorrect file or folder permissions first.

Ok. You say it might be related to a shipping mod? As a test, try disabling your live payment and shipping mods and turning on the cash on delivery mod and a different shipping mod such as weight based shipping. Put through a test order using those and see if that makes any difference. It's just a case of narrowing it down by elimination. I would put the site in maintenance mode while you test it.
