This team of doctors helps to coordinate services, provides quality care, and shares information so they can easily identify gaps in care and address them quickly. You may see a specialist in the Kelsey ACO at any time as referrals are not necessary.

There are three Kelsey-Seybold ACO plan options
The Basic plan
offers lower premiums than Plus options but has higher deductible and coinsurance amounts. This means you will pay more when you need health care. If you don’t visit the doctor often and use the plan mostly for preventive care, the Basic option may be right for you.
The Plus plan
has higher premiums than the Basic plan, and the deductible and coinsurance amounts are lower. This means more of your expenses will be covered when you need care. If you think you will visit the doctor often and need more care, the Plus option may be right for you.
The Select plan
has the lowest deductible and out-of-pocket maximum, but this option is only available to employees who make $29,000 or less in annual base salary in 2022 and $31,000 in 2023.

Memorial Hermann’s ACO is also a network of doctors and medical providers that share information across the Memorial Hermann system to provide care to covered members and their covered family members. This means everyone you see in the Memorial Hermann system is familiar with your treatment, needs, and goals, as well as your medical history. You may see a specialist in the Memorial Hermann ACO at any time as referrals are not necessary.

These are some of Aetna’s broad networks that provide more flexibility to members. You will need to elect a Primary Care Physician (PCP) or one in your zip code will be selected for you. You may see a specialist in the Texas Medical Neighborhood plans without a referral but selecting a PCP will allow a medical professional to coordinator your care.
